Granite VNA launches virtual nurse program 

CONCORD, N.H.Granite VNA recently introduced a virtual nurse program, connecting patients with skilled home and hospice care nurses through telephone and video calls. The virtual nurses complement the agency’s field nursing team by conducting regular check-ins with their patients, reinforcing education, addressing patients’ physical and mental health needs, supporting case management and more. 

Granite VNA currently has three virtual visiting nurses, two who specialize in home care and one who specializes in hospice care. Similar to the agency’s field nurses, the virtual visiting nurses see multiple patients each day. 

“Virtual visiting nurses play a crucial role in enhancing community health and providing support to Granite VNA’s home and hospice care field nurses,” said Granite VNA Vice President of Education and Quality Tammi Mozier, MBA, PT, ATC, COS-C. “By providing personalized care directly to patients through phone and video calls, the virtual visiting nurses improve the patients’ access to essential medical services and enable Granite VNA’s field nurses to manage their schedules more efficiently and continue delivering high-quality care to their patients.” 

The virtual nurse program offers crucial support to those with life-threatening or unstable medical conditions, such as congestive heart failure, chronic digestive disorders, diabetes or end-stage renal disease, providing essential aid and reassurance through remote, yet personalized, care. 

With regularly scheduled virtual visits, the goal is to educate and support patients and their families in managing their health at home, where they are most comfortable. Additionally, the calls provide patients with another private way to ask questions and share information regarding their medical needs.
